• Guardian information: Name, email, phone, address

• 2 Emergency contact’s information: Names, phone numbers, addresses, relationship

• Payment information

• The signed consent form: DOWNLOAD FORM, sign, and email it to inquiries@ritchie-league.com or drop off at the hall in our secured mail drop next to front door.
